4-Point inspection form

The 4-Point inspection form is a standardized insurance form that evaluates four specific systems of a home: roofing, electrical, plumbing, and HVAC. Insurance companies typically require it for homes 30 years old or older before issuing or renewing a homeowner's insurance policy. Here is how the form works in Inspector Studio.

When you need a 4-Point

A customer or their insurance agent will request a 4-Point. It is most common in Florida and other states where insurance carriers require documentation of older home systems before binding coverage. The 4-Point is not a full home inspection — it covers only the four systems listed above.

Creating a 4-Point report

1. Open the booking.

2. In the Reports section, click Create Report.

3. Select 4-Point Inspection as the report type.

4. Click Create.

The 4-Point form opens in the report editor. Unlike a standard home inspection report, the 4-Point has a fixed, standardized layout — you cannot add or remove sections.

Filling out the form

The form is divided into four sections, one for each system:

Roofing

• Roof covering material (shingle type, tile, metal, etc.)

• Estimated age of the roof covering

• Estimated remaining useful life

• Condition rating

• Evidence of leaks or damage

Electrical

• Type of electrical service (circuit breakers vs. fuses)

• Amperage of the main panel

• Wiring type (copper, aluminum, knob-and-tube, etc.)

• Any deficiencies noted

Plumbing

• Supply pipe material (copper, CPVC, PVC, galvanized, polybutylene, etc.)

• Drain pipe material

• Water heater age, type, and condition

• Any deficiencies noted

HVAC

• Heating system type and estimated age

• Cooling system type and estimated age

• Condition of each system

Photos on a 4-Point

Photos are required for a complete 4-Point submission. Insurance carriers typically want photos of:

• Each elevation of the roof

• The main electrical panel (open, showing breakers)

• The water heater (nameplate visible showing model and serial number)

• The HVAC equipment (nameplate visible)

Attach photos to each relevant section in the form using the photo attachment tool.

Signatures

The 4-Point form does not require customer signature — it is a professional report signed off by the inspector. When you are satisfied with the form, publish it. See Publishing a report for the publication steps.

Submitting to the insurance carrier

The 4-Point is typically delivered as a PDF to the customer or their agent, who submits it to the insurance carrier. Use the Share button on the published report to copy the link or download the PDF.
