WDO inspection form

A WDO (Wood-Destroying Organism) inspection report — commonly called a termite inspection report — documents evidence of wood-destroying organisms found at a property. These include termites (subterranean and drywood), wood-decaying fungi, and wood-boring beetles. The form follows a standardized state form and is typically required by lenders or real estate contracts as part of a home purchase.

Who can perform a WDO inspection

WDO inspections must be performed by a licensed pest control operator or a licensed home inspector with the appropriate WDO certification, depending on your state. Make sure the inspector performing the WDO has the required license before creating this report type.

Creating a WDO report

1. Open the booking.

2. In the Reports section, click Create Report.

3. Select WDO as the report type.

4. Click Create.

The form opens with the standardized WDO report layout for your state.

What the form covers

Visible evidence of WDO

For each type of wood-destroying organism, the inspector marks whether there is:

• No visible evidence — nothing found.

• Visible evidence — active or past infestation observed. The inspector documents the location and type.

• Previous treatment — evidence of prior treatment (chemical, bait, tenting, etc.).

Areas inspected

The form records which areas of the property were accessible and inspected:

• Exterior perimeter

• Interior accessible areas

• Attic (if accessible)

• Crawlspace or subfloor (if accessible)

Areas that were not accessible are documented as inaccessible with a reason.

Findings detail

For any positive findings, the inspector documents:

• Location (specific area of the property)

• Type of organism

• Whether infestation appears active or inactive

• Recommended treatment (informational — not the inspector's responsibility to treat)

Diagrams and photos

Many state WDO forms require a diagram of the property showing where findings were located. Photos of any evidence found should also be attached to support the findings.

Completing the form

When all fields are complete, publish the report. The WDO form is typically delivered as a PDF — use the Share link or download the PDF from the published report.
